I can not take credit for the name. This site was named by Aaron Cains.

I took the basic VSS with power sword and bolter, cut off the bolter and
removed her left arm below her sleeve. I then cut the sword off the
left hand and cleaned the right hand until I got rid of the rough metal
left by cutting the bolter off. I then took the Combi-weapon from the
Dark Angel Commander Azrael and cut a small wedge out of the top back of
the weapon (to make it fit in the wedge between her power armor shoulder
pad and the sleeve). I pinned the sword into position and then did the
same with the Combi-weapon. Azrael has larger gloves than a sister, so
I used some Kneadatite to fill out her sleeve in the back and to hide
the small gap that was left between the parts.

It's not perfect as Azrael's hand is a little bit too big, and the
Combi-weapon is technically a Bolter-Plasma gun, but I think it'll paint
up well enough and looks pretty good.
